Key Responsibilities:
• Conduct comprehensive assessments of patients and their families
• Develop and implement individualized care plans
• Manage palliative emergency situations actively
• Administer medications and advanced wound care
• Educate families about the end-of-life process
Requirements:
• Minimum three years of community palliative care experience
• Registered Nurse in good standing with the College of Nurses of Ontario
• Knowledge of pain and symptom management techniques
• Strong critical thinking and communication skills
• Flexibility to work various shifts
