Description

Position Summary

Practices as a member of the multidisciplinary team, under the standards of The College of Nurses of Ontario, Accreditation Canada, the Ministry of Health, Ministry of Long-Term Care and policies of Deep River & District Health (DRDH). In relation to the North Renfrew Family Health Team, under the direction of the Manager of Clinical Services-Primary Care, supports the provision of primary health care services to patients.





Position Relationships

Reports directly to the Manager of Clinical Services of the area they are assigned to. Collaborates with staff, physicians, families and patients, as well as external care providers to deliver care in response to the identified needs of patients.

Duties:

- Practice as a member of the multidisciplinary team to provide quality clinical care and teaching to patients, residents, and their families.
- Provide health education/counseling, and follow-up care, and performing other procedures according to the College of Nurses of Ontario standards of nursing practice.
- Download and triage laboratory and Health Report Manager (HRM) results, alerting physicians and nurse practitioners if lab results indicate timely intervention.
- Maintain clinic inventory of medical supplies and vaccines
- Ensure the cleaning and maintenance of equipment and clinical exam rooms.
- Adheres to all policies and procedures.
- Supports and demonstrates the mission, vision and values of the organization.
- Other duties may be assigned and may not be limited to the scope of an individual department.

Qualifications:

- Current Registration with the College of Nurses of Ontario as a Registered Practical
- Nurse, and be a member in good standing required.
- Practical Nursing Diploma or equivalent education and relevant experience.
- Current certification in Basic Life Support (BLS).
- 12 Lead ECG Interpretation Course or equivalent preferred.
- Gentle Persuasive Approach certificate preferred.
- Experience related to primary care approaches included but not limited to immunizations, Well-Baby, pediatrics, etc.




- Education and training related to dementia and geriatric care preferred: included but not limited to Montessori, restorative care approaches, palliative care, etc.
- Demonstrated focus on Person-Centered Care
- Experience leading interdisciplinary teams preferred.
- Knowledge of current trends in nursing and health care required.
- Demonstrated exemplary documentation skills and holistic assessment skills required.
- Knowledge of applicable legislative and hospital/program policies required.
- Exceptional Attendance required.
- Must agree to adhere to the standards of behaviour of the organization.
