In this part time role, you will work with the interprofessional Family Health Team to assess patient needs and enhance access to community services. Your responsibilities will include preparing action plans, supporting referrals, and managing resources dedicated to patient welfare. Engage with diverse populations to address psychosocial challenges and ensure seamless care transitions. Key Responsibilities:
• Work with a Family Health Team to support patients • Assess needs and coordinate community resource referrals • Manage patient progress and develop care plans • Facilitate communication with external service providers • Conduct complex care rounds and case reviews Requirements: • Master's degree in Social Work from an accredited school • Registered with Ontario College of Social Workers • 3 years of relevant interdisciplinary experience • Understanding of community services and health care models • Excellent communication and problem-solving skills Leverage your social work background to improve patient outcomes and facilitate coordinated care for those requiring additional support.
