ABOUT THE ROLE:

Our Patient Care Coordinators (PCC) are key members of the clinic team and often the first point of contact for patients seeking care. As a Float PCC, your role will be to provide coverage across our BC clinics (Vancouver through Chilliwack) based on operational needs, higher volume physician days, sick and vacation coverage, etc.

In this dynamic, patient-facing role, you will help create a welcoming, respectful, and non-judgmental environment while supporting individuals on their recovery journey. Working closely with physicians and the interdisciplinary CATC team (such as Addictions Counsellor, Clinic Lead, Director, etc), you will help coordinate patient care, support the opioid agonist therapy (OAT) treatment program, and ensure patients receive the information and support they need. This role is ideal for someone who is compassionate, organized, and motivated to make a meaningful impact in the lives of people affected by substance use.

KEY RESPONSIBILITIES:

· Provide flexible clinic support: Travel between CATC clinics across the Lower Mainland (Vancouver through Chilliwack)



to support teams during higher-volume physician days or initiative implementation, and provide coverage for sick leave and vacations, helping ensure consistent, high-quality patient care.

· Create a welcoming patient experience: Greet and support patients in a respectful, non-judgmental manner, assisting with check-in, orientation within the clinic, and basic needs such as mobility support and comfort. Consistently go above and beyond to foster the CARE principles with patients, colleagues, and the community.

· Support clinical care: Assist the medical team by collecting vital signs, preparing patients for appointments, collecting specimens (urine), and supporting medication preparation.

· Ensure patient safety: Monitor patients for adverse reactions, respond to urgent situations including overdose or fainting, administer naloxone when required, and provide first aid while offering emotional support to patients experiencing distress.

· Maintain accurate records: Document patient interactions, prescriptions, referrals, and clinical activities in the electronic medical record (EMR) while maintaining strict confidentiality and compliance with clinic protocols.

· Help keep the clinic running smoothly and connected to the community: Prepare consultation rooms, maintain supplies and equipment, support infection control practices, collaborate with healthcare staff to ensure efficient operations, and participate in community events (such as Recovery Day BC) while building connections with local organizations to support patients and clinic growth.

WHAT YOU’LL NEED TO SUCCEED:

- Completion of a High School Diploma and/or GED or equivalent is required.
- A certificate in Medical Office Administration or Health Services is an asset.
- Minimum of 1 year of experience in Substance Use, Mental Health, Healthcare and/or related fields.




- Recent and acceptable Criminal Reference Check.
- Current First Aid and CPR Certification is an asset.
- Valid driver’s license and reliable vehicle required. $150/month travel stipend provided.
- Ability to manage stressful situations with professionalism and compassion.
- Capability to work both independently and collaboratively within a multidisciplinary team; showing skilled demeanor with initiative and accountability and empathy.
- Excellent interpersonal, communication and organizational skills.

LOCATION:

Float Patient Care Coordinator will work at our BC CATC clinics across the Lower Mainland (Vancouver through Chilliwack) schedule based on higher-volume physician days, operational requirements, sick coverage and vacation coverage. A primary home clinic location will be established with the successful candidate and may be Surrey or New Westminster.

HOURS (may vary based on specific clinic hours):

Monday: 9:30 AM – 6:00 PM

Tuesday: 9:30 AM – 5:00 PM

Wednesday: 9:30 AM – 5:00 PM

Thursday: 9:30 AM – 5:00 PM

Friday: 9:30 AM – 4:00 PM

Pay: $20.00-$21.00 per hour
