You will guide interdisciplinary teams while supporting holistic care for residents and families.

Key Responsibilities
- Complete assessments and support diagnostic testing procedures
- Order, interpret, and communicate diagnostic testing results
- Lead resident-centered palliative care services
- Educate and support multidisciplinary Village teams
- Participate in clinical reviews and quality initiatives

Requirements:
- Current Nurse Practitioner registration with the College of Nurses of Ontario
- Experience completing comprehensive geriatric assessments
- Current Heart Save Level CPR certification
- Current AIS certification
