Winnipeg Regional Health Authority is seeking an Integrated Program Wellness Coordinator within the My Health Team network. The role focuses on case management, collaboration with community clinics, and alignment of mental health strategies to support patient wellness.

The incumbent will report to the My Health Team Manager, work with inter-qualified teams, and help deliver Recovery Model services across community and clinical settings.
