About the Role

Senior Circle Support Inc. is seeking reliable Personal Support Workers (PSWs) to assist patients receiving peritoneal dialysis (PD) treatment in their homes throughout Durham Region.

Successful candidates will support our home PD program, working alongside the nursing team and following established care plans and protocols.

Responsibilities

- Assist with home peritoneal dialysis care within the PSW's authorized role and demonstrated competence.
- Support patients with PD-related preparation and non-nursing tasks.
- Follow infection prevention and safety protocols.
- Observe and promptly report changes in patient condition or treatment concerns.
- Maintain accurate visit documentation.




- Communicate effectively with nurses and the care coordination team.

Qualifications
- Recognized PSW certificate.
- Previous healthcare or home care experience.
- Experience supporting patients receiving PD is an asset.
- Willingness to complete program-specific training.
- Reliable transportation throughout Durham Region.
- Strong communication skills and ability to work independently.

Compensation and Scheduling
- $23 per hour
- Casual/part-time shifts.
- Morning, evening, and weekend availability preferred.
