Qualifications – Ontario Healthcare Facility
Current registration as a Nurse Practitioner with the College of Nurses of Ontario (CNO)
Minimum 2 years of relevant experience
BLS / CPR certification
Experience working in Long-Term Care is required
Full-time availability
Monday–Friday schedule
Start Date: ASAP
End Date: Permanent

Responsibilities:
Nurse Practitioners are required to adhere to the scope of practice as defined by the College of Nurses of Ontario. Responsibilities may include but are not limited to:
Provide comprehensive nursing and primary care to residents in a Long-Term Care setting
Conduct assessments and monitor residents' health status
Develop and implement individualized treatment and care plans
Assess and manage acute and chronic health conditions within scope of practice
Order and interpret diagnostic tests and laboratory results as required
Prescribe medications and treatments within scope of practice
Collaborate with physicians, nursing staff, residents, families, and other members of the interdisciplinary care team
Provide education and support to residents, families, and caregivers
Maintain accurate and timely clinical documentation




Follow applicable healthcare regulations, infection prevention practices, and patient safety standards

Note: Duties may vary slightly based on facility and shift, with specific guidance provided during site orientation.

Experience:
Minimum 2 years of relevant Canadian nursing or Nurse Practitioner experience
Long-Term Care experience required
Experience managing complex and chronic conditions is an asset
Ability to work independently and collaboratively within an interdisciplinary team
Strong clinical judgment and adaptability

Skills:
Excellent communication and interpersonal skills
Strong clinical assessment and decision-making abilities
Ability to prioritize tasks and manage time effectively
Strong documentation and clinical record-keeping skills
Compassion and empathy when supporting residents and families
Ability to manage a full time caseload in a Long-Term Care environment
