Reporting to the Manager of Continuing Care, you will assess, plan, implement, and evaluate nursing practices within a multidisciplinary setting. Key Responsibilities:
• Assess and evaluate nursing care for diverse patients
• Implement care plans in a multidisciplinary setting
• Provide post-surgical and cardiac patient support




• Communicate effectively with patients, families, and staff
• Maintain documentation adhering to skilled standards

Requirements:
• Current certification with the College of Nurses of Ontario
• Competent in Basic Cardiac Life Support (CPR)
• Adult medical/surgical nursing experience required
• Solid prioritization and communication skills
• Ability to work independently in a busy environment
