As a Social Worker, you will work independently within a multidisciplinary team, delivering care planning and referrals to clients facing social, emotional, and psychological barriers. Key Responsibilities:
• Conduct comprehensive client assessments and care planning
• Provide crisis and grief counseling to clients and families
• Liaise with health care professionals for coordinated care




• Educate caregivers and develop in-service content
• Manage and prioritize caseload in a complex setting

Requirements:
• Bachelor's degree in Social Work from a recognized institution
• Current BC College of Social Workers registration
• Two years of recent, related experience
• Valid BC Driver's License
• Working knowledge of community health services
