Collaborating closely with the Hospice Residence Manager and various healthcare professionals, you will assess needs, develop care plans, and ensure comfort and dignity for patients and their families during challenging times.
Key Responsibilities:
• Conduct comprehensive assessments of patients and families
• Develop individualized care plans aligned with patient goals
• Administer medications and advanced wound care
• Manage palliative care emergencies independently
• Educate patients and families on their conditions
Requirements:
• Must have a valid RPN registration with the College of Nurses of Ontario
• Minimum three years of experience in community palliative care
• Flexibility for rotating shifts, including nights and weekends
• Knowledge of pain and symptom management
• Excellent communication and critical thinking skills
