Collaborate with an interdisciplinary team to create and maintain accurate resident assessments, care plans, and documentation. Key Responsibilities:
• Collaborate with care team for personalized care
• Conduct resident assessments and maintain care plans
• Implement, evaluate, and adjust care plans as needed
• Educate residents and families on health conditions
• Respond to medical emergencies while managing medications
Requirements:
• Current LPN registration in the applicable province
• Experience in senior care or long-term care preferred
• Solid clinical knowledge and communication skills
• Current CPR and First Aid certification
• Willingness to complete Vulnerable Sector Check
