Make a meaningful impact as a Community Care Navigator at Dorothy Ley Hospice, working full-time with an equity-informed, person-centered approach. Support individuals in navigating complex health systems while ensuring timely intake and administrative coordination.
As a key member of the Community Care team, you’ll manage general administration and coordinate intake processes, assisting both healthcare professionals and the community. You will conduct initial screening for palliative, caregiver, and bereavement needs, while also reducing barriers to care. Collaborate closely with program staff and volunteers to enhance service delivery and support community members effectively.
Key Responsibilities:
• Process new referrals accurately and timely in EMR
• Serve as the primary contact for inquiries from health professionals
• Conduct registration and intake screenings for community referrals
• Provide navigational support for health and social services
• Support volunteer program administration and coordination
Requirements:
• BSW or equivalent experience in social work
• Minimum two years with individuals facing life-limiting illnesses
• Proficient with electronic health records
• Robust interpersonal and communication skills
• Experience in hospice or health care settings preferred
