Cota is looking for a dedicated Mental Health Case Manager to provide collaborative support to clients in need. This permanent full time position focuses on assessing clients’ needs and developing individualized care plans.
As part of the Toronto East Integrated Service Team, the Case Manager will work full-time to support individuals with mental health and cognitive challenges. This role requires strong skills in client assessment, advocacy, and service coordination to help achieve client goals. You will join a dynamic team dedicated to improving clients' wellbeing through personalized assistance and engagement.
Key Responsibilities:
• Review referral information and determine program eligibility
• Develop supportive therapeutic relationships with clients
• Complete comprehensive client assessments collaboratively
• Assist clients in creating personalized Client Safety and Recovery Plans
• Facilitate access to community resources and other supports
Requirements:
• Post-secondary degree/diploma with 2-3 years relevant experience
• Advanced assessment/support skills for target populations
• Knowledge of recovery-oriented service delivery principles
• Strong communication and crisis management skills
• Proven ability to work effectively in a team
