At CarePartners, we are seeking a certified Nurse Specialized in Wound, Ostomy & Continence to deliver essential care to patients in clinics and the community. Your role includes promoting self-management among clients and leading interdisciplinary teams to achieve the best patient outcomes. Additionally, you will receive extensive orientation and ongoing clinical support, along with the necessary tools for success.
Key Responsibilities:
• Provide wound and ostomy consultations in various settings
• Develop personalized care plans and assessments
• Utilize evidence-based practices for patient safety
• Collaborate with healthcare teams on treatment goals
• Perform thorough patient assessments and provide education
Requirements:
• RN diploma with current CNO registration
• NSWOC certification or equivalent experience
• Several years of experience in wound management
• Exceptional communication and interpersonal skills
• Valid driver’s license for travel within the region
