Complete in-home functional assessments and mobility evaluations
Develop individualized, evidence-based treatment plans




Recommend assistive devices and environmental modifications
Provide follow-up visits for progressive functional goals
Educate clients and families to promote independence Requirements:
Registered with the College of Occupational Therapists of Ontario
Experience in home and community care preferred
Solid skills in functional assessments and documentation
Ability to work independently within service area
