Summary of Duties
As a member of the Assertive Community Treatment Team, the incumbent will provide client centred treatment, rehabilitation, support, and case management to clients in the community with severe & persistent mental illness.

Qualifications / Skills

MSW, B.Sc. OT, M. Sc. OT, or COAT approved Equivalent required

Member in valuable standing of the Ontario College of SW and SSW, or College of Occupational Therapist of Ontario required

Recent community experience working with patients with severe and persistent Mental Illness

Knowledge and comfort with functioning within the ACT model of Care

Basic knowledge of psychotropic medications

Ability to work independently and interdependently within a multidisciplinary team

Excellent oral and written communication skills

Demonstrated effective interpersonal skills

Demonstrated problem-solving skills

Commitment to client-centered care

Knowledge of the health care system and community resources

Valid Ontario Driver’s License required

Ability to travel extensively within the catchment area
