Key Responsibilities:
• Collaborate in case planning with clients and service providers
• Advocate for client needs, including housing issues
• Observe and assess client behavior in emergencies
• Provide training on life skills like budgeting and hygiene
• Document and maintain client care plans and reports
Requirements:
• At least two years of relevant post-secondary education
• Minimum two years’ experience in mental health services
• Knowledge of psychosocial rehabilitation and crisis intervention
• Experience assisting individuals at risk of homelessness
• Successful completion of a criminal record check
