Position Summary
The Community Health Team Lead supports the Community Health Program Manager in coordinating, supervising, and delivering community health programs and services to MBQ members. This includes oversight of the Community Health Representative Program, Medical Transportation, Non-Insured Health Perks (NIHB), Infection Prevention and Control, and Community Health Nursing programs.
The Team Lead supervises Community Health staff in the planning and delivery of health promotion and disease prevention initiatives within the MBQ community and Quinte Mohawk School. They help ensure services are responsive to current and emerging health needs and are delivered in a timely, culturally appropriate, and effective manner.
Working collaboratively with internal teams, community partners, and external service providers, the Team Lead coordinates and advocates for accessible health services for eligible clients. The role promotes community engagement in improving overall health and wellbeing, encourages individual responsibility for preventative health practices, and supports the ongoing development and effectiveness of the Community Health Team.
Summary of Main Responsibilities
Assessment & Treatment
- Apply principals of health care delivery, frameworks, case management, collaborative practice models
- Responds decisively to emerging opportunities, risks, and changing health needs
- Ensures high-quality community health nursing care and health programs are delivered
- Promotes awareness of community health nursing programs and related services
- Incorporates diverse approaches to health and wellness, including traditional and/or alternative practices, alongside western and mainstream community health care practices
- Provides individual and group oversight, coaching, and mentoring to Community Health Program staff.
- Fosters a supportive, respectful, and collaborative team environment
- Ensures compliance with safety protocols,
infection prevention and control measures, and community health best practices
- Acts as an interpreter/liaison, between community members and other CWC health care delivery teams
- Liaises with off-reserve community health service providers and partners
- Assumes managerial responsibilities in the absence of the Community Health Program Manager.
- May provide direct client care and/or services as operational needs require
Administrative Functions
- Assists in the development, implementation, and ongoing review of Community Health policies
- Maintains current knowledge of departmental budgets, funding agreements, and reporting requirements
- Identifies opportunities for program development and funding
- Assists in the recruitment, supervision, performance evaluation, and professional development of team
- Collaborates in research, evaluation, and data collection activities
- Participates in quality improvement initiatives, discussions, and continuous improvement activities
- Identifies service gaps and emerging needs within Community Health programs and nursing practices
- Conducts audits of client files and nursing care
- Supports team admin functions, including scheduling, timesheets, budgets, funding and reporting
- Participates in meetings, conferences, seminars, and ongoing professional development and practice
Compliance & Safety and All Other Duties as Assigned
Requirements
A bachelor's degree in nursing (BSN) or RPN Nursing Diploma
Minimum of 3 years related nursing work experience, including the supervision and / or coordination of personnel
Experience in client education and support
A combination of education, training and experience would be considered
Current membership in good standing with the College of Nurses of Ontario and valid certificate of registration with the RNAO
Updated immunization records and First Aid / CPR Certificate with AED; ongoing renewal as required to maintain current certification
Clear Vulnerable Sector Check
Valid Class “G” driver’s license with clean abstract and reliable method of transportation.
Previous experience working in an Indigenous setting would be considered an asset
Ability to work flexible and extended hours and participate on call if necessary.
Knowledge, Skills and Abilities:
Knowledge and understanding of Indigenous culture, traditions, teachings, community dynamics an asset
Knowledge of Employee Development and performance management
Strong clinical assessment skills
Excellent patient care skills to provide patients with compassionate and comprehensive care
Excellent proficiency in Microsoft Office (Word, Excel, PowerPoint, Teams, and EMR system).
