Working in lockstep with health system partners, our skilled team supports people and their families throughout their care journey.If you are aRegistered Dietitianwho wants to be part of bringing integrated care to Toronto and are passionate about caring for patients in their care journey, we want you on our team!Job DescriptionAssesses the dietary unique to each person and family, considering the social determinants of health, ethnicity, culture, gender, sexual orientation, language, religion, age, ability and their preferencesAssesses the person’s and caregivers’ skills and need for dietary education and supportsCreates a holistic, person-centred plan, acknowledging the psychosocial impact of changing nutritional requirements and dietary intakeProvides education, advice and practical strategies to the person and caregivers regarding the management of dietary related symptoms, including anorexia, cachexia, dysphagia, nausea, vomiting, constipation, diarrhea and taste changes, management of enteral and parenteral nutrition supportEngages in dysphagia assessment and managementCollaborate with interdisciplinary teams, including but not limited to physicians, nurse practitioners, nurses, personal support workers, social workers, speech therapists, occupational therapists,
