This role oversees key initiatives including the Family Transition Program, Adult Day Program, and transitional services for admissions, ensuring alignment with the home’s mission, values, and person‑centered care philosophy to enhance quality of life and inclusion.QualificationsMinimum five (5) years of experience in gerontology, long‑term care, or community program delivery.Demonstrated experience in staff supervision, program coordination, and leadership.Knowledge of applicable legislation, including the Fixing Long‑Term Care Act, Resident Bill of Rights,
