Employment Type: Full-time, Short-term contractDepartment: Seniors ServicesUnion: Included (CUPE)Salary: $29.87/hr (Level 3)File #: 26-SCY-22Posting Date: June 04, 2026Posting Close: June 11, 2026Reason for vacancy: ReplacementPosition SummaryReporting to the Manager, Comprehensive Intake and Care, the Intake Coordinator is responsible for the overall tasks involved with client intake conducting interRAI CHA pre-screener assessments, communicating effectively with internal and external resources, collecting statistical information, maintaining an accurate, relevant database. The position also provides backup support in the transportation program. The Intake Coordinator is required to perform their duties in a manner consistent with Dixon Hall’s Mission, Vision and Values, ensure safety, and demonstrate customer service excellence. The working hours are Monday to Friday: 9:00 AM to 4:00 PM.Duties and ResponsibilitiesProgram DeliveryReceive and review referrals & requestsScreen and assess clients’ eligibility for services and programs; provide consultation regarding appropriateness of servicesProvide information to clients, caregivers, community members and service providersDirect referrals to appropriate programs & services in the department or other community support agencies as necessaryComplete interRAI CHA pre-screener and basic intake assessmentsReview and/or check the RMR system and Dove Care for referralsCollaborate with program coordinators to initiate serviceCoordinate with other partner organizations and internal resourcesDepartment outreach,
networking and program developmentAdministrationEstablish and maintain statistical records which track referral sources and services/support to which clients are referredComplete digital intake documents and create initial client file in databaseCompleted intake and client file is triaged and communicated to appropriate program staffProvide information for funder reports as necessaryOrganizational ResponsibilitiesWork collaboratively with other Dixon Hall staff and departments and volunteersOperate in an effective, efficient and client focused mannerActively participate in department and agency meetings and eventsWork under the guidance of the agency and program policies and procedures, including the Mission, Vision and Values of Dixon Hall and the agency’s strategic direction.Develop positive relationships with referring agencies, outreach engagements and inter-agency networking tablesMeet regularly with manager to discuss individual, team and program effectivenessUndertake other tasks and responsibilities as assigned within the purview of the job descriptionQualificationsPost-Secondary education in Social Services, Gerontology or a related fieldA minimum of 3 years’ experience working with vulnerable or high-risk populations including seniors, adults with disabilities and/or with mental health issues, and individuals who are underhoused.Excellent communication skills (written & verbal).Strong assessment skills.Knowledge of community-based programs and servicesAbility to work independently and co-operatively both in consultation & jointly with others.Solid organizational and planning skills.Flexibility.A second language is a definite asset.Understanding and knowledge of the housing and health sector and services.Ability to exercise positive judgment, flexibility, creativity, and sensitivity to changing situations and needs.Intermediate level of computer literacy utilizing MS Office applications.Knowledge of AlayaCare will be considered an asset.First Aid/CPR certification will be considered an asset.Must be qualified to work in Canada legally for the employer.Able to provide a Vulnerable Sector Screening check with a ‘Clear’ or satisfactory result that meets the operational requirements of the Agency.#J-18808-Ljbffr
