The Edmonton Southside Primary Care Network was the first PCN in Alberta. Since 2005 the ESPCN has grown to become the largest PCN in Edmonton and consists of a team of health care professionals who are passionate about creating healthier communities. Position Overview:

The Edmonton Southside Primary Care Network has an exciting opportunity for a temporary 0.6-0.8 FTE, Exercise Specialist (ES). The ES will provide comprehensive physical activity services and care to patients within the Primary Care Network. The ES will assess, plan, implement, and evaluate physical activity care plans to patients of all ages.

The ES will practice to the full extent of the knowledge, skills and competencies required for the ESPCN ES role, with a focus on physical activity assessment, exercise prescription, behaviour change support, and chronic disease prevention and management.

The ES will work with the family physicians in the community, other health professionals and local and regional programming in a multidisciplinary environment to ensure a high quality of evidence-based health care to the practice population. This role will work out of our Central ESPCN Clinic. The successful candidate will need to be flexible to be available to work a minimum of one evening shift a week. This is a 12-month contract.





The Exercise Specialist will report to the Primary Care Manager-Central Clinic Sr. Manager, centralized services.

Key Roles and Responsibilities:

- To work collaboratively in a patient-centred, team-based primary care practice setting.
- To participate in the care of individual patients, practice populations and associated communities.
- To assess patients’ baseline physical activity and readiness for change, prescribe exercises, provide exercise supervision, health education and follow-up evaluation as needed.
- To make professional, competent, and autonomous decisions with patients as a member of the multidisciplinary team.
- To act as a physical activity resource to other multidisciplinary team members, healthcare institutions and community agencies or programs.
- To ensure accurate, objective and timely clinical documentation in the appropriate medical record.
- Facilitation and supervision of physical activity groups.

Experience, Skills & Qualifications:

- Baccalaureate degree in Physical Education, Kinesiology, or equivalent
- Canadian Society of Exercise Physiology Certified Exercise Physiologist (CSEP-CEP) or American College of Sport Medicine Clinical Exercise Specialist (ACSM CES) Certification
- Current Basic Life Support (BLS) for Healthcare provider
- Vehicle/license – when appropriate

- Previous experience
- -Primary care setting-Electronic Medical Record-Group facilitation/instruction-Health behavior change methods (eg Motivational Interviewing)




- Understand the Alberta privacy laws and act according to those laws.
- Should behave responsibly, be accountable and have excellent written and verbal skills, problem solving/critical thinking skills.
- Experience facilitating group classes.
- To support the vision and mission of the Primary Care Network.
