Qualifications

- Grade 12 education (MB Standard)
- Post-secondary education in a health/social service field
- Experience relevant to working with diverse backgrounds including cultural, financially disadvantaged, and/or socially disadvantaged
- Knowledge and experience working with community groups
- Knowledge of the Employment and Income Assistance Program (EIA), Community Volunteer Income Tax Program (CVITP), Old Age Security (OAS), Guaranteed Income Supplement (GIS), Pharmacare (and Hydro/Pharmacare program), Child Tax Credit, and other financial assistance programs
- Knowledge and experience regarding the broad range of factors that serve to keep communities healthy
- Province of Manitoba Class 5 Drivers Licence, or equivalent from province of residence, and access to a personal vehicle to provide service within Prairie Mountain Health
- Proficiency with Microsoft programs (Outlook, Word, Excel, Access and PowerPoint), as well as Internet applications and other Information Technology
- Demonstrated organizational skills, and the ability to work independently
- Demonstrated communication skills
- Demonstrated flexibility to facilitate changes in techniques and procedures
- Ability to display independent judgment
- Ability to respect and promote a culturally diverse population
- Ability to respect and promote confidentiality
- Ability to perform the duties of the position on a regular basis

Position Summary The Financial Facilitator will work as part of My Health Team, a cooperative partnership between independent primary care practices, community organizations and Prairie Mountain Health.



The Financial Facilitator will be responsible for the delivery of income security focused interventions to support and address the health needs and priorities of an identified target population within the My Health Team area. The aim of the position is to work with and support client groups impacted by the challenge of poverty, complex lifestyles, and chronic disease.

The Financial

Facilitator will connect clients with existing income security services, bridge gaps in financial support services, and build capacity to effectively address financial vulnerability as a key component of managing chronic disease.

Responsibilities

Overview

Working collaboratively with My Health Team providers and other relevant partners:

- Acquires and maintains up-to-date detailed knowledge of financial issues and financial resources relevant to people with limited income.
- Develops relationships with relevant resources and community members/groups to promote effective resource utilization.
- Implements targeted programs/sessions such as banking basics, income tax submissions, social services forms, EIA application processes, budgeting, establishing good credit, preparation for employment, conducting an employment search, etc. for populations with limited income.
- As requested, assists individual clients and families with income security focused interventions.
- Offers information sessions for internal and external partners.
- Participates in the collection of evaluation data.
- Provides reports to meet My Health Team requirements.
- Promotes respectful and de-stigmatizing language.
- Other duties as assigned.
