Over the past 17 years, the ESPCN has grown to become the largest PCN in Edmonton and consists of a team of health care professionals who are passionate about creating healthier communities. We are currently seeking a full-timePrimary Care Managerto join our team!Primary Care Manager is responsible for the implementation of strategic and operational objectives of the PCN across medical homes through management of multidisciplinary teams, relationship development with members, and leadership and engagement in medical home optimization efforts. Reporting to the Senior Manager - Central Clinic and working in collaboration with the management team, the Primary Care Manager leads the day-to-day implementation of PCN programs and services within their clinic portfolio. The Primary Care Manager collaborates with the Quality Improvement team and Clinical Educator team towards medical home optimization and team-based care within clinics. Primary Care Managers may also be assigned program leadership responsibilities and function as a Program Lead.People Leadership and Team DevelopmentProvides overall leadership to clinical teams, including coaching and mentorship, providing feedback, and goal-setting and evaluation. This includes regular 1:1 meetings and completion of the annual performance management and review cycle.Identifies individual and/or team skills gaps and/or competency gaps and implements coaching and mentoring measures to enhance skill development and service delivery.Identifies staffing resources required, and collaborates with HR to participate in recruitment, including interviewing and selection of candidates.Collaborates with HR in dealing with employee relations matters as required within their clinics/staff portfolio, including development and implementation of performance improvement plans to address performance concerns.In collaboration with Clinical Educators, oversees orientation and onboarding of new clinical team members and leads their integration into clinic teams.Program Leads provide program or discipline support to other Primary Care Managers as required.Leads and fosters employee and team engagement to ensure a culture of teamwork, collaboration, and service orientation. This includes modeling appropriate leadership behaviour.Oversees initial staff scheduling and works collaboratively with other PCMs to ensure workload coverage and highest and best use of staff.Provides input into resource allocation from both a financial and HR perspective to the managers of finance and HR and Clinical Director. Program Leads will provide input on specific discipline and program areas.Manages human resource administration such as day-to-day scheduling, vacation requests, coverage,



and payroll / time management for direct reports.Makes professionally autonomous and principle-based decisions to enhance program integration in clinics.Chairs monthly discipline meetings, committee meetings, and team meetings, as indicated.Program Development & ImplementationParticipates and, where appropriate, leads in the development, implementation, and evaluation of appropriate, efficient, and valued PCN programming such as clinical programs and quality improvement initiatives.When assigned as a program lead, leads a specific discipline or program across all clinic portfolios. Examples include nursing, dietician services, and mental health. This includes developing, implementing, and evaluating programs aimed at enhancing primary care services in the lead area. In addition, this role is also responsible for oversight of front-line delivery of these lead services in all clinics.Develops and implements relevant and necessary protocols and procedures required for consistent service delivery.Provides input into broad program/services operational plans.Identifies the necessary resources and recommendations related to program areas to the Clinical Director.Collaborates with evaluation team to ensure evaluation of all programs and services in their respective clinics are appropriate. In the Program Lead role, there is an additional responsibility of providing input into these specialty areas. Initiates and implements or supports new or modified approaches, practices, or processes based on evaluation outcomes.Intervenes and/or investigates incidents within the clinics that may impact patient and/or patient or staff safety.Provides input/feedback to Clinical Director related to system integration needs and challenges. Supports and participates in system integration and collaboration initiatives at the clinic and team level.In the Program Lead role, there is a greater expectation to stay abreast of health care trends, policies, legislation, emerging best practices, AI, and its impact on program development and decision-making.ManagementWorks collaboratively and effectively with other Primary Care Managers and clinic teams to ensure that all processes are consistent across the teams and mitigates duplication of effort between teams.Responsible for ensuring alignment with organizational policies, health care regs, standards and best practices, and providing input into quality standards and program eval. Within their clinics.Responsible to be the primary link between members and clinics and the PCN. They will facilitate services and support for their clinics, keeping them informed of PCN offerings, services, and opportunities.Ensures the maintenance of accurate records and documentation related to decision-making and critical communications.Serves as a key liaison in relation to programs or services both with member clinics and individual members, other PCNs, community partners, AHS/PCA, discipline associations. In this capacity, promote awareness of programs and/or services supporting clinics and other stakeholders to provide enhanced program capabilities,



expand services and/or improve patient outcomes.Point of contact for all issues / concerns of patients for which the team is responsible and in resolving service delivery issues with stakeholders.Recognizes opportunities for change across their assigned clinics and/or program services areas and brings forward those recommendations to the Clinical Director.In collaboration with the Finance & Administration Manager, manages annual budget projections and expenditures.Rotating duty as expenditure officer.Provides support and coverage for other managers when they are absent.Leads or provides input into special projects and/or other duties as assigned (eg. Committees).Baccalaureate degree in a recognized health discipline combined with a minimum of 2 years in a leadership/managerial/supervisory/mentorship role and 4 years of previous professional experience in healthcare delivery or administration, with preference given to those with primary or community care experience.Current Registration with applicable College regulated under the Health Professions Act of Alberta(preferred)Experience in some or all of the following areas is desirable:Primary Care/Community health careMental HealthChronic Disease (i.E. Diabetes, Cardiovascular disease, COPD/Asthma); health promotion and preventative health.Preference for individuals who have supervised other clinical disciplines.Knowledge of scope of practice and related legislation for Registered Nurses, Licensed Practical Nurses, NPs, Registered Dietitians, Social Workers, and Exercise Specialists.Working knowledge of process improvement, change management and various quality improvement tools and methodologies. Change management and quality improvement training/education would be a strong asset (examples: ProSci, Six Sigma,, IHI).Knowledge of best practices in health administration and/or healthcare program delivery and quality improvement principles and practicesAbility to make evidence-based decisions and monitor clinical standardsAbility to make principle-based decisions based on operational and strategic objectivesShould behave responsibly, be accountable and have excellent written and verbal communication skills, problem solving and critical thinking skillsAbility to navigate ambiguity and manage conflictKnowledge, skill and proficiency with Microsoft Office 365 and electronic medical recordsUnderstand confidentiality and the Alberta privacy laws and act according to those laws and internal policiesOther RequirementsCurrent CPRMust have own vehicle and valid driver’s licenseFlexible work hours including occasional weekends and eveningsHow to Apply?If this sounds like the role for you, please apply with your resume and cover letter.
