Position Title: Mental Health Counselor / System Navigator

Reports To: Executive Director

Wage: $35.60 - $40.60

Position Summary The Mental Health Counselor/ Navigator is responsible for working with physicians and other members of the Family Health Team to assist individuals with mental illness and psychological issues.

The Mental Health

Counselor/ Navigator will communicate with other mental health agencies in the community to ensure that patients are referred to appropriate programs and services. Duties and Responsibilities

Operational

System Navigation/Brief Support a) Provide Intake and initial assessment of patients’ mental health and social needs; determine the most appropriate support plan in collaboration with the patient and other care providers.

b) Provide brief, solution-focused counselling to patients and families, as needed.

c) Provide practical assistance to patients in identifying and navigating community-based mental health and support resources.

d) Work collaboratively with LWA FHT Mental Health Clinicians and other agencies for collaborative care planning and facilitating smooth transitions.

Counseling e) Assess patients’ mental health needs and provide treatment for cognitive, emotional, behavioural disturbances by psychotherapeutic means.

f) Facilitate group counselling.

g) Develop,



maintain and facilitate workshops and ongoing groups for wellness and positive mental health. Collaborate and consult with community groups and other health care agencies to develop new public education programs and identify and fulfil community health care needs.

h) Support the use of progress and outcome measurement tools.

i) Support the use of assessment tools that other members of the Family Health Team can use to assess the mental health condition of patients.

j) Remain current with the latest mental health research and trends.

k) Create and/or update patient charts to document patient progress.

l) Prepare documents, records, and reports as required.

Health and Safety a) Personally comply with and assume appropriate supervisory responsibility for compliance with all health and safety practices in the organization; in accordance with policies, standard operating procedures/guidelines, and the Occupational Health & Safety Act and applicable regulations.

b) Promote and support a culture of patient safety and advocacy.

Other a) Comply with all policies and procedures of North Perth Family Health Team.

b) Perform all other duties and responsibilities as assigned.

Education, Skills and Experience

Education/Training/Certifications

- Master’s Degree in Social Work, Psychology preferred, or equivalent education/experience




- Registration with a college whose members are eligible to practice psychotherapy under the Regulated Health Professions Act, 1991 (e.g. OCSWSSW, CRPO, CNO, COTO)

Experience
- Minimum one year of related experience.

Skills and Abilities:
- Thorough working knowledge of social work/psychology principles and practices.
- Excellent critical thinking and problem-solving skills.
- Strong interpersonal and customer service skills.
- Exceptional organizational and verbal and written communication skills.
- Ability to demonstrate tact and discretion in handling confidential and/or sensitive matters; maintain confidentiality.
- Knowledge of Microsoft Windows and Office applications.
- Ability to effectively foster good rapport and cooperative working relationships; and to champion the corporate mission and values.

Demands and Working Conditions Work is typically performed in a standard office setting. Extensive computer work including keyboarding, concentration, data entry, and sitting for extended periods of time, is required.

The Mental Health Counselor/ Navigator is required to make decisions and solve problems using judgement and interpretation. Frequently exposed to human suffering and loss of life. Interaction with clients that may exhibit violent or harassing behaviour, have special needs, or mental illness.

Job Type: Full time

Pay: $35.60-$40.60 per hour
