The role involves working with adults and seniors with mobility disabilities in York, Sunbury, Carleton, and Victoria counties. The counsellor meets with clients at home, in hospital, or at the rehabilitation centre, acting as counsellor, planner, navigator, and facilitator to empower independence and community participation.ResponsibilitiesMeet with clients at home, in hospital, or at the rehabilitation centre to assess needs and goals.Develop action plans, identify solutions, and mobilise supports and services.Collaborate with clients and their support networks to promote independence and community participation.Serve as a counsellor, planner, navigator, and facilitator.QualificationsPost‑secondary degree in Psychology, Sociology, or Human Services.Experience in counselling and person‑centred planning.Excellent organisational, time‑management, communication, relationship‑building, writing,
