Nurse Practitioner - Full Time Permanent .80FTE – Filling a Vacancy

Organization: Lakehead Nurse Practitioner-Led Clinic (LNPLC)

Location: Thunder Bay, Ontario

Salary range: $103,752.22 annually (may qualify for Northern Community Program for Nurses Funding) along with extended health care benefits, HOOPP pension (optional), learning and developmental opportunities

Hours of work: Monday to Friday, 8:30 am – 4:30 pm (may include some evenings and weekend hours)

Position Summary:

We are seeking a compassionate, dedicated Nurse Practitioner to join our multidisciplinary team on a full time .80 FTE basis.

The successful candidate will provide high-quality, patient-centered primary care while working collaboratively with Nurse Practitioners, physicians, community paramedics, and other healthcare professionals. The NP will contribute to comprehensive patient assessment, diagnosis, treatment, health promotion, disease prevention, and chronic disease management.

The NP will provide community-based primary health care in collaboration with the Lakehead Superior Connect program to patients living with complex chronic conditions, including COPD, congestive heart failure, and frailty. Working closely with community paramedics, the NP will support ongoing assessment, monitoring, and management, with the goal of identifying changes in health status early, optimizing treatment and quality of life, and reducing avoidable emergency department visits and hospitalizations.

The Lakehead Nurse Practitioner-Led Clinic (LNPLC)



is a primary care clinic focused on providing high-quality, patient-centered healthcare in Thunder Bay, Ontario. Our team of Nurse Practitioners, along with other healthcare professionals, work collaboratively to deliver comprehensive care to individuals in the Thunder Bay area.

Key Responsibilities:

- Assess, communicate, diagnose and prescribe treatment and treatment plans with an emphasis on health promotion and disease prevention.
- Collaborate with our LNPLC team and in the development of the patient plan of care, and to develop and implement strategies that promote health and prevent illness.
- Work closely with community paramedics to support ongoing patient assessment for complex chronic conditions
- Advocate and facilitate the principles of self-care to patients and team members.
- Act as a resource for patients, staff, and other healthcare professionals.

Key Responsibilities Continued:

- Practice in accordance with College of Nurses of Ontario Practice Standards and LNPLC expectations.
- Collaborate with other healthcare team members to ensure an integrated approach to patient care.

Education, Qualifications and Skills:

- Primary Health Care NP (PHC-NP) Certificate with registration in the Extended Class, with the College of Nurses of Ontario.
- Experience in nursing assessment skills in primary care or community health setting is preferred.
- Demonstrated interpersonal, communication, problem solving, and leadership skills.




- Excellent communication skills and the ability to build strong relationships with patients and the healthcare team.
- Experience with electronic medical records system is an asset.
- Proficient in use of computers and information technology.
- Ability to work independently and as part of a team.
- A commitment to ongoing professional development.
- Current Ontario Driver’s License
- Current BCLS
- Up to date on vaccinations (including MMR and Covid) – must provide immunization record
