The Grand Bend Area Community Health Centre (GBACHC) is seeking a compassionate, collaborative, and motivated Nurse Practitioner (NP) to join our interdisciplinary primary care team on a full time basis.

Position Summary The NP provides comprehensive, evidence-informed primary care across the lifespan at our Grand Bend and Hensall sites. Working with our interdisciplinary team, the successful candidate will support health promotion, disease prevention, and the management of acute and chronic diseases, as well as coordinate access to community-based care.

Employment Details

- Position Type: Full-time, 1.0 FTE
- Location: Grand Bend Area Community Health Centre, Grand Bend and Hensall locations
- Annual Salary: $137,000

Qualifications

- Master of Nursing and Nurse Practitioner certificate from a recognized university, or equivalent education that meets College of Nurses of Ontario (CNO) requirements
- Current registration and in good standing with the CNO in the Extended Class (RN-EC)
- Primary care, community health, hospital, or public health nursing experience; primary care experience is an asset




- Knowledge and experience related to mental health, addictions, and chronic disease management are considered assets
- Strong clinical judgment, organization, communication, and ability to work independently and collaboratively within an interdisciplinary team
- Strong computer skills and experience with electronic medical records (EMRs); PS Suite EMR experience and familiarity with local resources are assets

Key Responsibilities Clinical & Professional Practice

- Provide comprehensive primary care across the lifespan, including assessment, diagnosis, treatment, and follow-up for acute and chronic conditions
- Order and interpret laboratory and diagnostic tests, prescribe medications, make referrals, and coordinate care within the NP scope of practice
- Provide preventive care, screening, health education, counselling, and health-promotion support
- Develop, implement, and monitor individualized care plans in partnership with patients and the interdisciplinary team
- Maintain timely documentation in the EMR and support continuity of care
- Maintain professional competency, confidentiality, ethics, and accountability standards

Team & Community Engagement

- Promote the mission, vision, and values of the GBACHC




- Collaborate with team members and community partners and participate in case conferences
- Contribute to program development, evaluation, continuous quality improvement, and student learning opportunities
- Serve as a clinical resource and contribute to responsive access to care

- Support a safe, respectful, and inclusive environment for patients and staff
- Adherence to Occupational Health and Safety legislation, organizational policies, and safe clinical practices

What We Offer The GBACHC values teamwork, innovation, equity, and patient-centred care. We offer:

- Competitive compensation: $137,000 annually for 1.0 FTE
- Time away from work: Four weeks of vacation and 15 personal/sick days, annually
- Professional development: Ten days annually, plus $500 for professional development and $500 for related travel
- Comprehensive benefits: Health and dental coverage and participation in HOOPP
- Work-life balance: Shared Saturday coverage approximately once every five weeks, with a full lieu day the following week

Vacation, personal/sick time, professional-development days, and allowances are prorated in the year of hire.

Working Conditions

This role is based in a standard office and clinical environment at the Grand Bend and Hensall sites. Flexible scheduling is required to meet organizational and patient needs, including the shared Saturday morning walk-in clinic rotation.

Pay: $137,000.00 per year
