Silver Fox Pharmacy in Burlington is seeking a full time Pharmacist to join our Long-Term Care pharmacy operations team.
We are looking for a pharmacist who is dependable, collaborative, organized, and comfortable working in a busy team-based environment. The successful candidate will work closely with pharmacy staff and other healthcare professionals to support the safe and accurate provision of medications to residents in long-term care.
Key Responsibilities
- Practice in accordance with all applicable provincial and federal legislation, professional standards, and Ontario College of Pharmacists requirements
- Collaborate with pharmacists, pharmacy technicians, assistants, and other members of the pharmacy team to ensure medications are processed and dispensed safely and accurately
- Review prescriptions and medication orders for therapeutic appropriateness and technical accuracy.
- Identify potential drug-related problems, discrepancies, or other concerns and communicate with prescribers or other healthcare professionals when clarification or intervention is required
- Work collaboratively with nursing staff and prescribers to support medication reconciliation and preparation of the Best Possible Medication History (BPMH) for newly admitted residents
- Maintain complete, accurate, and organized resident medication and health records
- Document clinical interventions and other relevant medication-related information appropriately
- Support efficient pharmacy workflow and assist with resolving medication-order or operational issues as they arise
- Maintain current knowledge of pharmacy practice, medications, professional standards, and regulatory requirements through continuing education
- Prioritize workload effectively and manage competing responsibilities in a fast-paced environment
- Contribute positively to a collaborative and supportive pharmacy team
Qualifications
- Registered and in good standing with the Ontario College of Pharmacists
- Strong communication, organizational, and time-management skills
- Ability to work effectively and collaboratively with pharmacists, pharmacy technicians, nurses, physicians, and other members of the healthcare team
- Strong attention to detail and commitment to medication safety
- Experience in Long-Term Care pharmacy is an asset
- Experience with Kroll Windows is an asset
- Familiarity with electronic medication administration record (eMAR) systems is an asset
