Overview

We are seeking a dedicated and detail-oriented Primary Care Nurse to join our team. The nurse is responsible for assisting and supporting medical providers in delivering high quality primary health care to patients of Courtyard Medical Clinic. The ideal candidate will play a crucial role in ensuring the smooth operation of the clinic by providing comprehensive nursing care within an interprofessional primary healthcare team.

The nurse will be supporting preventative care, chronic disease management, patient education, care coordination, and clinical operations to improve patient outcomes and access to care. Working collaboratively with physicians, nurse practitioners, allied health professionals, and community partners, the RN promotes evidence-informed, patient-centered care.

Qualifications

Education:

- Bachelor of Science in Nursing (BScN) required.
- Current registration in good standing with the provincial nursing regulatory body.

Experience

- Experience in primary care, family medicine, community health, or ambulatory care preferred.




- Experience managing chronic diseases and complex patient populations.
- Proficiency with electronic medical records (EMRs).

Knowledge and Skills

- Strong clinical assessment and triage skills.
- Knowledge of chronic disease management and preventative care guidelines.
- Excellent communication and patient education abilities.
