The Salvation Army is hiring a Case Manager to support residents at Gateway. The role involves direct consultation, referral, supportive case management, and community resource coordination.Service ResponsibilitiesProvide direct consultation, referral, and supportive case management to residents of the shelterConduct intake interviews with first time and ongoing residents to determine needs and offer appropriate supports and referralsProvide case management services to residents at Gateway in accordance with THHS case management strategy and as required by supervisorAssist residents in establishing goals, monitoring progress, and preparing case plan toward housingRefer residents to appropriate resources for the purpose of housing, employment, educational, recovery and other case specific needsDocument all relevant case notes and maintain confidentiality and accuracy of case filesParticipate in regular case management meetings and other meetings as requiredUtilize community resources and cultivate where possible useful community contactsRepresent Gateway at community meetings as appointed by the supervisorPerform all related duties as determined by the Supervisor of Case ManagementOrganizational ResponsibilitiesAdhere to standards that are consistent with the values and philosophy of The Salvation Army and The GatewayComply with all Health and safety requirements and other relevant legislationParticipate fully and constructively in staff meetings and in-service trainingEngage in formal evaluations and supervision with supervisorRead and follow the Policy and Procedures Manual and the Employee Handbook as well as read and sign off on reading Shelter Standards and adhere to the standards in dealings with Residents, Clients, Staff and AdministrationBecome familiar with the provisions of the position descriptionAs required and applicable, make suggestions that will improve efficiency, working conditions and procedures to the managementRepresent the Gateway in a skilled mannerEssential Skills And Qualifications2 years of College Social Services education and/or Psychology diploma or certificateMinimum of two year experience in the field of Mental Health or AddictionsNOTE: An alternative level of education and experience may be acceptable.Additional QualificationsGood working knowledge of community resourcesGood command of English language and interpersonal skillsCertification in First Aid/CPR and Crisis intervention trainingClean criminal records checkHostel Standards Training Certificate an assetCompensationThe target hiring range for this position is $24.47 to $30.59 with a maximum of $36.71.
