REGISTERED NURSE - existing position

SUMMARY The Smiths Falls Nurse Practitioner-Led Clinic (NPLC) provides exemplary primary health care services to the residents of Smiths Falls and the surrounding counties of Leeds, Grenville, and Lanark.

Under indirect supervision, and working as part of an interdisciplinary team, the Registered Nurse works to full scope and provides high quality professional nursing care to assigned clients, consistent with the vision of the Smiths Falls NPLC.

ACCOUNTABILITY The Registered Nurse reports to and is responsible to the management team. All clinical decision making outside the scope of the Registered Nurse must be done in consultation with the Smiths Falls NPLC NP Lead/medical directives. The Registered Nurse must comply with the standards of practice guidelines of the College of Nurses of Ontario.

ROLES AND RESPONSIBILITIES

-Performs nursing assessments, nursing treatments, health education/counseling and other clinical activities according to CNO standards of practice for RNs

-Responsible for overseeing and administration of immunization programs offered at the Smiths Falls NPLC, in conjunction with other staff

- Responsible for ensuring that the Smiths Falls NPLC maintains highest level of infection control practices in all areas
- Participates in prevention and chronic disease management through education, monitoring, and instruction of self-management techniques
- Integrates appropriate community resources in education processes
- Contributes to the development of program planning and develops, plans, implements, coordinates, and evaluates assigned projects in consultation with other team members
- Supervises nursing students and participates in the education of other students
- Ensures that electronic encounter forms and other statistics have been recorded
- Contributes to workflow evaluation processes

- Attends continuing nursing education

SPECIFIC DUTIES




- Maintains appropriate stock of all vaccines
- Performs immunization of all clients, all ages
- Prenatal and well-baby and well-child visits
- Assessing clients for acute episodic visits
- Participates in targeted immunization programs i.e., flu clinics

- Oversees Occupational Health Program
- Ensures clinic is following appropriate infection control practices
- Other clinical duties such as desensitization, Vitamin B12, BP clinics, etc.
- Manages client health education resources including prenatal packages
- Performs ECGs and holter monitoring
- Works collaboratively with team, including other RN(s) through role sharing
- Other duties as assigned

QUALIFICATIONS

- Diploma or Bachelor of Science in Nursing
- Must be a member in good standing with the Ontario College of Nurses
- In depth knowledge of the local health program delivery and community-based services
- In depth knowledge of chronic illness care, including both clinical and supportive care
- Ability to prioritize, manage time effectively and be flexible in an active work environment
- 2+ Years’ experience working in primary care setting an asset
- Ability to operate with the highest level of confidentiality and discretion
- Strong interpersonal skills and communication skills
- Ability to work flexible schedule
- Ability to travel for home visits in Smiths Falls area
- Demonstrated computer skills
- French speaking an asset
- Certified Diabetes Educator an asset
- RN prescribing an asset
