: Registered Practical Nurse (RPN)

The Registered Practical Nurse (RPN) plays an important role in providing safe, compassionate and high-quality nursing care to our residents. Through clinical knowledge, leadership and person-centred care, the RPN supports residents' health, comfort, dignity and quality of life while working collaboratively with residents, families and the interdisciplinary care team.

The Leamington Mennonite Home is a faith-based long-term care and retirement community founded in 1965 to support seniors in aging with dignity. Today, we serve 273 residents across a range of housing and care options.

Qualifications

- Current registration in good standing as a Registered Practical Nurse with the College of Nurses of Ontario (CNO).
- Current CPR and First Aid certification.
- Experience working in long-term care, retirement living, gerontology or a related healthcare setting is an asset.
- Knowledge of applicable nursing standards, medication administration, infection prevention and control, documentation and resident safety practices.
- Knowledge of person-centred care principles and experience supporting individuals with dementia and responsive behaviours is an asset.
- Strong clinical assessment, critical-thinking and decision-making skills.
- Excellent communication, interpersonal and organizational skills.
- Ability to work independently within scope of practice while collaborating effectively with the interdisciplinary care team.
- Ability to provide leadership, direction and support to Personal Support Workers and other members of the care team.
- Good physical health and fitness appropriate for the position and the ability to safely perform the physical requirements of resident care.

Areas of Responsibility

- Provide safe, compassionate and resident-centred nursing care in accordance with residents' individual plans of care.
- Assess, monitor and respond to residents' physical, emotional, cognitive and behavioural needs within the RPN scope of practice.
- Administer medications and treatments safely and accurately in accordance with physician orders,



organizational policies and applicable professional standards.
- Provide leadership, guidance and direction to Personal Support Workers and other care team members during assigned shifts.
- Foster positive and professional relationships with residents, families, physicians, volunteers and colleagues.
- Work collaboratively with Registered Nurses, Personal Support Workers, physicians, Recreation, Dietary, Housekeeping and other interdisciplinary team members.
- Contribute to a respectful, safe and supportive environment that reflects LMH's mission and values.

Job Duties

- Complete nursing assessments and monitor residents for changes in health status, responding and escalating concerns appropriately.
- Administer medications and treatments and monitor residents for therapeutic effectiveness and potential adverse reactions.
- Provide wound care and other nursing treatments within scope of practice and in accordance with resident care plans and physician or authorized prescriber orders.
- Maintain accurate, timely and complete clinical documentation.
- Participate in the development, implementation, evaluation and updating of individualized resident plans of care.
- Communicate changes in resident condition to the appropriate healthcare provider, Registered Nurse, Charge Nurse, Director of Care, Nurse Practitioner and Physician as well as family or substitute decision-maker as required.
- Work collaboratively as well as provide direction and support to Personal Support Workers to ensure resident care is delivered safely and consistently with established plans of care.
- Respond to resident health concerns, incidents and emergencies and initiate appropriate interventions within scope of practice.
- Support residents and families with compassion, professionalism and clear communication during routine care, changes in condition and challenging circumstances.
- Promote resident independence, choice, dignity and participation in decisions affecting their care.
- Follow infection prevention and control practices, medication-management procedures, workplace safety requirements and emergency protocols.
- Participate in medication reviews, quality-improvement activities, audits and other clinical initiatives as required.
- Maintain professional competence through ongoing education and continuing professional development.




- Ensure appropriate communication and continuity of care during shift changes through accurate reporting and handover.
- Demonstrate professionalism, accountability, teamwork and pride in the workplace.
- Perform other related nursing duties as assigned within scope of practice.

Compensation & Schedule · Starting wage: $33.17/hour; After 1750 hours $34.07; After 3500 hours $35.02

· Shift premiums: Evening/nights = 85 cents; W/E = $1.10

- Mix of 8 hour and 12-hour shifts in both Long Term Care and Retirement Residence
- Must be available to work all 3 shifts and weekends.
- Employment is subject to a 450-hour probationary period.
