Registered Practical Nurse (RPN) Job Summary

We are seeking a compassionate, professional, and dedicated Registered Practical Nurse (RPN) to join our multidisciplinary team. The successful candidate will provide high-quality nursing care, support clients throughout their treatment journey, and collaborate closely with physicians, psychiatrists, pharmacy partners, and other healthcare professionals to ensure exceptional client outcomes.

Key Responsibilities

- Practice in a professional, ethical, and accountable manner in accordance with the standards established by the College of Nurses of Ontario (CNO).
- Deliver safe, client-centered nursing care while maintaining the highest standards of clinical excellence.
- Administer and dispense medications, including injections, as prescribed and authorized by licensed physicians.
- Collaborate and communicate regularly with physicians, psychiatrists, and other healthcare professionals regarding client health, treatment plans, and progress.
- Coordinate and schedule medical and psychiatric appointments for admissions, medication management, follow-up care, and discharge planning.
- Participate in the development, implementation, coordination, and evaluation of individualized client care plans as part of an interdisciplinary treatment team.
- Maintain accurate, confidential, and up-to-date documentation within the Electronic Medical Records (EMR) system.
- Liaise with pharmacy partners to ensure continuity of care and effective medication management.
- Complete comprehensive nursing assessments for new admissions, including health histories, medication reviews, vital signs, and other clinical assessments.
- Assess and respond to client health concerns, provide wound care when required, and complete incident reports and clinical documentation as necessary.
- Accompany clients to medical appointments, specialist consultations, or pharmacy visits when required.
- Monitor and maintain medication inventories and medical supplies to ensure adequate stock levels.




- Participate actively in team meetings, case conferences, training sessions, and organizational initiatives.
- Adhere to all organizational policies, procedures, professional standards, and regulatory requirements.
- Perform other duties as assigned to support client care and organizational operations.

Qualifications
- Current registration in good standing with the College of Nurses of Ontario (CNO).
- Current CPR and First Aid Certification.
- Strong clinical assessment and documentation skills.
- Experience working in mental health, addiction treatment, residential care, or community healthcare settings is considered an asset.
- Excellent communication, organizational, and interpersonal skills.
- Ability to work independently and collaboratively within a multidisciplinary team.
- Proficiency with Electronic Medical Records (EMR) systems.

Pay: $27.00-$30.00 per hour
