In this pivotal role, you will work with individuals and their families to create personalized, goal-centered programs. Key responsibilities include assisting with physical, emotional, and psychological needs, while maintaining a supportive home workplace. You will collaborate with a dedicated team to ensure high-quality care and must be empathetic, resourceful, and skilled in both documentation and medication administration.

Key Responsibilities:
• Develop individualized care plans with Service Users
• Ensure cleanliness and organization of residences




• Maintain accurate logs and documentation
• Manage medication dispensing and orders
• Transport Service Users as needed

Requirements:
• Diploma in Developmental Social Work or Human Services
• 2 years' experience with adults with disabilities
• First Aid and CPR certification required
• Current non-violent crisis intervention training
• Valid Ontario Class “G” Driver’s license
