This full time permanent position requires you to engage individuals experiencing precarious housing or homelessness, leveraging your outreach expertise to foster trust and enable access to essential services. You will work collaboratively within a multidisciplinary team, ensuring a comprehensive approach to clients' health needs.

Key Responsibilities:
Provide direct outreach and case management for clients
Document and maintain up-to-date client health records
Build strong relationships with community partners and agencies




Facilitate access to resources and support for clients
Represent outreach team at local health forums and tables

Requirements:
Education in social services or related disciplines
Minimum three years of relevant community experience
Solid collaboration and communication abilities
Knowledge of health inequities and social determinants
Willingness to work flexible schedule and travel
