PALLIATIVE CARE CLINICAL COACH – 095-26-3Home and Community Care, WellbeingContract (12 months)Hours of work: 35 hrs./weekWage: Min. $43.08/hrJOB SUMMARYThe Palliative Care Clinical Coach reports to the Home and Community Care Manager and works under the direction of the Six Nations Wellbeing Department. The role drives practice change and improves access and quality of care across community organizations. The position collaborates with Six Nations community palliative care, Ontario Health, the Ontario Palliative Care Network, and related frameworks.KEY DUTIES & RESPONSIBILITIESTechnical Functions:Provide palliative care coaching, mentoring, and clinical guidance to staff and clinicians to strengthen primary-level palliative care competencies.Consult on complex clinical care situations when patient needs exceed the current competencies of care providers.Collaborate with local health leadership, the palliative care network, and community partners to support change management and quality improvement initiatives.Coordinate and facilitate education and training aligned with the Ontario Palliative Care Competency Framework and local community needs.Work with the Palliative Care Case Manager and partner teams to support assessment, care planning, evaluation, and ongoing development of the palliative care program.Evaluate the quality of palliative care services across participating organizations, identify service gaps, and help guide quality improvement planning.Carry out responsibilities in accordance with the standards of the College of Nurses of Ontario and within the scope of practice of a regulated health qualified.Administrative Functions:Co-create and support implementation of the regional Delivery Framework plan with Ontario Health, including measurement, reporting, and practice change activities.Maintain accurate records and support data collection, reporting, and review of progress and key learnings.Develop and support a palliative care framework that builds capacity, advances quality improvement, and informs future hospice and model-of-care planning.Help ensure staff have access to the education, training,



and mentorship needed to perform their roles effectively within the palliative care program.Administer training activities within approved budget parameters and participate in regular meetings with management, case management, the palliative care team, and volunteer staff.Ensure community implementation remains aligned with Ontario Palliative Care Network guidance and contribute to relevant advisory groups, committees, working groups, and task forces.Communications Functions:Build and maintain effective relationships with Ontario Health, community organizations, advisory groups, subject matter experts, and other key partners to support collaboration and trust.Lead engagement and community-building activities with participating organizations and support broader system integration.Communicate key messages clearly to diverse audiences and develop presentations and materials that explain palliative care concepts, implementation priorities, and program plans.Monitor community service needs, identify challenges and opportunities, and keep implementation partners informed.Liaise with local and external service providers to support care coordination, integration of resources, and smooth client transitions.Attend regional and local planning meetings, care rounds, and related forums to support service planning and clinical updates.Provide program information to the public, provider agencies, and community groups as required.Other Functions:Participate as a member of the Home and Community Care Program, including the Palliative Care Outreach Team and Care Partners.Attend seminars, conferences, workshops,



and professional development activities to remain current on evolving health and social issues related to palliative care.Support the Six Nations Emergency Measures Plan by maintaining awareness of emergency procedures and providing assistance as directed.Perform other related duties as assigned by the Six Nations Home and Community Care Program Case Manager.WORKING CONDITIONSWork requires extensive physical activity; requires confidentiality; requires travel and much interaction with the public; subject to interruptions and unscheduled hours; ability to take and seek direction and work independently with little supervision.WORKING RELATIONSHIPSWorks within the services of Home & Community Care, Case Management, and the Palliative Care Committee. Received direction, instruction and supervision, interacts to ensure tasks are done efficiently and effectively.KNOWLEDGE AND SKILLSA Registered Nurse or Nurse Practitioner with experience in palliative care. Minimum five (5) years experience practice in palliative care is required.Knowledge of relevant legislation and reporting requirements including the Compassionate Care Act.A member in good standing with the College of Nurses of Ontario.Must have a vehicle and valid vehicle insurance. Must have a class “G” Driver’s license.Must relate effectively to and supervise staff.High level of computer skills – Microsoft Office. Be willing to train on electronic charting.Will be willing to work flexible hours.Will have working knowledge of relevant legislation, regulations and policies. Will be familiar with relevant local and area resources.Will be trained in First Aid/CPR.Will have good communication and public relations skills. Must relate effectively to and supervise staff.Must effectively manage financial matters.Must work within a multi-disciplinary team setting.Must function independently and initiate action as required.Must communicate clearly and effectively, both written and verbal. Highly sensitive to Six Nations Culture and working in client’s homes.BASIC QUALIFICATIONSA Registered Nurse or Nurse Practitioner with experience in palliative care; minimum five (5) years of experience in practice.Knowledge of relevant legislation and reporting requirements including the Compassionate Care Act.Member in good standing with the College of Nurses of Ontario.Vehicle and valid vehicle insurance; class “G” Driver’s license.Strong supervisory and staff relation skills.#J-18808-Ljbffr
