Reporting to the Area Manager, Red Deer Home Care, you will interact with the team and clients/families to provide quality care to clients in the community. As part of the Intensive Home Care and Community Response team you will work closely with clients/families and members of the health care team to facilitate early hospital discharge and to prevent hospital admissions when appropriate. The successful candidate will play a key role in timely and appropriate referrals to stakeholders as relevant and/or provide direct care as needed. Required Qualifications:
Completion of an accredited practical nurse education program.Active or eligible for registration and a practice permit as a Licensed Practical Nurse with the College of LPNs and HCAs of Alberta (CLHA).Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
Additional Required Qualifications: Completion of an accredited Licensed Practical Nurse education program.Active or eligible for registration and practice permit with the College of Licensed Practical Nurses of Alberta (CLPNA).Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).Minimum 2 years’ experience in Home Care or related experience required.
Preferred Qualifications: I.V certification preferred. Experience in HPT, wound care, palliative care and client teaching is an asset. Excellent communication skills and the ability to work independently in a team environment are needed.For Internal candidates, completion of Required Organizational Learning and up to date CCHSS education.
