In this temporary role, you will report to the Manager of Mental Health Services. The focus is on providing essential outreach services to elderly patients facing complex mental health, addictions, or neurological conditions. Your experience in geriatric assessment and interdisciplinary teamwork is vital to supporting both patients and their families.

Key Responsibilities:
• Conduct psycho/social and mental status assessments
• Offer counselling, support, and advocacy services
• Implement geriatric services including psychiatry and behaviour support




• Collaborate with family members and community providers
• Maintain excellent organizational and communication skills

Requirements:
• Current registration with the College of Nurses of Ontario
• Two years' recent experience in geriatric assessment
• Familiarity with DSM-IV-R and Mental Health Act
• Valid Class G driver's license and travel willingness
• Strong computer skills in Excel and Word
