Primary Care Alberta is seeking an LPN to serve as a Chronic Disease Management Nurse within the Northwest PCN. You will educate patients, monitor disease panels, and work closely with physicians to deliver secure, family-centered care. Eligible for northern allowance and travel reimbursement, supporting service in northern Alberta.
The role requires four years of LPN experience, strong communication, and willingness to travel to rural communities. Connect Care familiarity is a plus.
