Your responsibilities will include health assessments, crisis intervention, and medication management. Key Responsibilities:
• Assess and manage clients' bio-psycho-social care needs
• Conduct health assessments including crisis evaluations
• Complete physical health assessments and treatments




• Collaborate with interdisciplinary teams on care planning
• Provide health education and client medication management

Requirements:
• Practical Nursing Diploma from a recognized institution
• Registered Practical Nurse with the College of Nurses of Ontario
• Two years of experience in mental health or addictions
• Valid Ontario Driver’s License and reliable transportation
• Current First Aid and CPR certification
