Provides direct supervision, leadership, and education to other members of the health care team. Acts as a resource and advocate for residents, families and staff. Promotes the values, goals and philosophy of the Home. Promotes a safe and secure environment for all residents.Assessment of WoundsAssess wounds as per the Skin Care and Wound Management ProtocolEnsure that the skin risk assessment is completed on admission, including head‑to‑toe assessmentAssess/educate residents with actual skin breakdown weekly in WoundRounds and upload into PCCAssess residents re‑admitted from the hospital or returned from a leave of absence > 48 hours with skinbreakdownCoordinate with the health care teamCoordinate with Assistant Director of Nursing if a referral to an Enterostomal Therapist Consultant is requiredInform the Assistant Director of Care to order or rent a therapeutic surface as necessaryProvide the dietician a monthly list of residents with skin breakdown and access to WoundRoundsCoordinate with physician and the ET consultant any resident(s)
that needs to be referred to a Plastic SurgeonAssist the Registered Staff with dressing of complex wounds as per MD ordersQuality Improvement/DocumentationEnsure a weekly entry in PCC progress notes through WoundRounds of the resident’s wound status when residents have altered skin integrityEnsure treatment orders on Physician’s order sheets, eMAR, and WoundRounds are updated and accurateCheck that Care Plans are up to dateAnalyze data collected of wound prevalence in WoundRoundsEnsure quarterly head‑to‑toe assessments are completedUnderstand quality indicators used by the Home and participate in their improvementEducationCoordinate with Director of Nursing/Assistant Director of Nursing or Staff Development Coordinator on education on skin and wound managementResponsibilities to Health and SafetyBe aware of employee responsibilities and follow all Health and Safety policies and proceduresWork safely to reduce risk of injury to self, co‑workers and residentsBe alert to and report all actual or potential hazardous situations to immediate supervisorBe aware of WHMIS regulated products and take safety precautions accordinglyDo not operate or use faulty equipmentPromptly report personal injury to immediate supervisor and seek first aid as neededRender first aid as requiredHave a good understanding of the "Worker’s Responsibilities" under the Occupational Health and Safety ActBe aware of how and when to use personal protective equipment (PPE)Attend all mandatory health and safety training sessionsReport hazards to your Supervisor and the JHSCUnderstand and follow all Home health and safety policies and proceduresGeneralOther duties as assignedQualificationsCurrent Certificate of Competence with the College of Nurses of Ontario as Registered Nurse or a Registered Nurse registered with a regulatory authority in a Canadian jurisdiction other than Ontario, holding the equivalent of a certificate of registration authorizing independent practice in Ontario as of right exemption Bill 60Basic CPRExperience in long‑term care setting is an assetAbility to work with and relate to interdisciplinary team membersAbility to work from the heart and make others smileWell‑developed written and oral communication skillsKnowledge of RAI‑MDS – AIS certified; if not, must be completed within one year of hiringMust have a explicit vulnerable sector screen, no older than 6 months, TB test results less than 6 months old, and be able to provide three supervisory referencesCOVID‑19 vaccination is a condition of employmentPhysical DemandsA physical demands analysis is required by an employer to determine whether an employee has the medical ability, critical strength, and mobility to safely work in a specific job.
