This role in the HomeBase program requires a dedicated professional to assist formerly homeless individuals. You will employ intensive case management to develop personalized care plans and support clients in navigating community resources. The position is a full-time six-month contract. Key Responsibilities:

- Assist individuals transitioning from homelessness to permanent housing
- Complete assessments to evaluate needs and risks
- Support clients with daily living skills and crisis intervention
- Collaborate with team members and community partners
- Transport clients and advocate for their rights

Requirements:

- Bachelor’s degree in social work or related field
- Experience in the homeless-serving system
- Solid understanding of Housing First and Harm Reduction
- Solid written and oral communication skills
- Basic First Aid certification required
