St. Joseph's Health System is seeking a Community Support Counsellor to be part of a multidisciplinary team for the Haldimand Norfolk region. The right candidate will utilize best practices in psychosocial rehabilitation while providing crucial support and addiction counseling.

You'll engage individuals with severe mental health issues and help facilitate their successful integration into community living. Key Responsibilities:

- Provide crisis intervention and rehabilitation services
- Conduct assessments and implement treatment plans




- Support individuals with ongoing mental health challenges
- Collaborate with multidisciplinary teams for holistic care
- Utilize best practices in psychosocial rehabilitation Requirements:
- Post-secondary education in Psychology, Sociology, or related field
- 1-2 years’ experience with chronic mental illness
- Knowledge of harm reduction principles
- Familiarity with the ACT Model
