Improving Dementia Care Program (IDCP) Implementation Lead

Annual Salary - $63,190.50 - $78,988.13 (commensurate with experience)

Temporary Full Time (6 Month Contract)

St. Joseph’s Health Centre is seeking an IDCP Implementation Lead to support the successful implementation of the Improving Dementia Care Program (IDCP) model of care at Villa St. Gabriel Villa.

This temporary position provides dedicated project leadership throughout the implementation process, supporting staff education and engagement, facilitating interdisciplinary collaboration, and promoting culture change and person-centred, emotion-based dementia care.

The IDCP Implementation Lead works closely with residents, families, leadership, physicians, nurse practitioners, interdisciplinary team members, Behavioural Supports Ontario (BSO), and other partners to support the successful adoption of the IDCP model and establish sustainable practices beyond the implementation period.

Key Responsibilities

- Lead the implementation of the approved IDCP model of care in collaboration with organizational and site leadership.
- Develop, coordinate, and monitor the IDCP implementation plan, timelines, deliverables, and milestones.
- Facilitate the IDCP Working Group and support effective interdisciplinary collaboration.




- Engage staff throughout the implementation process through education, coaching, communication, and ongoing support.
- Promote culture change and the adoption of person-centred, emotion-based approaches to dementia care.
- Support staff in applying evidence-informed dementia care practices within their day-to-day work.
- Monitor implementation progress, identify barriers, and develop practical strategies to support successful adoption.
- Collect, analyze, and report project data, outcomes, and required information.
- Collaborate with residents, families, staff, leadership, physicians, nurse practitioners, BSO, and external partners.
- Communicate project progress, emerging issues, successes, and recommendations to appropriate leadership.
- Identify opportunities to improve implementation processes and support continuous quality improvement.
- Develop and contribute to sustainability planning to ensure successful practices are maintained following completion of the implementation period.
- Maintain accurate and confidential project documentation in accordance with organizational policies and applicable privacy legislation.
- Use project resources and organizational equipment responsibly and assist with monitoring project expenditures, as assigned.




- Promote a safe setting through evidence-informed dementia care practices and compliance with organizational health and safety requirements.

Qualifications

- Degree or diploma in a regulated health profession, health sciences or a related field.
- Minimum three years of experience in Long-Term Care, geriatrics, dementia care, or a related healthcare environment.
- Experience leading interdisciplinary initiatives and implementing practice or culture change is preferred.
- Additional education or certification in dementia care, quality improvement, project management, change management, or related areas is considered an asset.
- Demonstrated knowledge of person-centred and evidence-informed dementia care practices.
- Strong project coordination, organizational, and time-management skills.
- Excellent communication, facilitation, coaching, presentation, and relationship-building skills.
- Demonstrated ability to engage and influence staff and interdisciplinary teams through periods of change.
- Strong critical thinking, problem-solving, and decision-making skills.
- Demonstrated ability to identify implementation barriers and develop practical, sustainable solutions.
- Proficiency with Microsoft Office applications and the ability to collect, analyze, and present project data.
- Knowledge of applicable Long-Term Care legislation, privacy requirements, and organizational policies is considered an asset.
