Lead integrated health services as the Integrated Care Manager at Durham Community Health Centre. As a key member of the management team, you will report to the Vice President of Integrated Care and Experience.

The Manager ensures delivery of high-quality, client-centered services while supervising interprofessional staff and promoting a culture of collaboration. Your leadership will enhance program integration and support the strategic objectives of the Durham Community Health Centre. Key Responsibilities:

- Implement strategic and operational plans aligned with DCHC goals
- Supervise interprofessional teams, championing collaboration and accountability




- Monitor program performance against financial and operational targets
- Foster internal referrals for coordinated client-centered care
- Ensure compliance with DCHC standards and funder requirements

Requirements:

- Undergraduate degree in health care or related field
- Minimum 5 years in community-based health organizations
- Experience in integrated care delivery and performance management
- Robust knowledge of health equity and determinants of health
- Excellent communication and organizational skills
