Summary:
Reporting to the Jordan’s Principle Supervisor, the Jordan’s Principle Navigator provides service coordination for First Nations children and their families by helping them access health, education and social supports through Jordan’s Principle. The Navigator serves as a knowledgeable resource, contacts Indigenous Services Canada on behalf of children and families, and helps navigate federal, provincial/territorial and community programs and services to address a child’s needs.
The role supports consistent, comprehensive and culturally appropriate service delivery in alignment with Chippewas of the Thames First Nation strategic planning, policies, procedures, standards and best practices. This is a full-time position based on funding approval. The successful candidate must be able to work some evenings and weekends and travel as required.
Responsibilities:
· Apply all Department of Indigenous Services Canada program and service requirements to the Jordan’s Principle Program.
· Ensure the Jordan’s Principle Program applies consistent, comprehensive, competent and culturally appropriate services.
· Deliver, design and coordinate a comprehensive outreach strategy and Jordan’s Principle information sessions; promote Jordan’s Principle to families, communities and service providers; encourage children with unmet needs and their families to secure access to needed services and supports; and submit requests to Indigenous Services Canada on behalf of children and their families.
· Ensure staff adherence to organizational policies, procedures and standards.
· Support a culture of learning and professional development and ensure leadership development opportunities exist for staff.
· Ensure financial policies and procedures are followed.
· Administer the Aboriginal Children’s Health and Well-Being Measure survey in the school/community to assess the physical, mental, emotional and spiritual wellbeing of children and youth.
· Coordinate with the mental health team to ensure wellness checks are completed before and after the survey with children and youth.
· Build and nurture relationships across community-based programs and services, service providers, and First Nations, federal, provincial and territorial programs and services; identify and develop possible service-delivery models that improve timely access to services for First Nations children living in or outside their communities.
· Ensure children receive quality and culturally appropriate services and supports; where families require assistance, help identify service providers that support children’s access to health, education and social services across all stages and levels of care.
· Support data collection to better understand the scope of children’s needs and the nature of service gaps, including distribution of annual client surveys provided by Indigenous Services Canada.
· Perform other duties as assigned by the Health Director.
Qualifications:
· A Degree in Social Work/Social Service Work in accordance with the Province of Ontario, including registration in good standing with the College of Social Work.
· Three to five years of direct experience working with children and families using various therapies and models, including Holistic World View and culturally appropriate therapies.
· Experience working with Indigenous people and organizations, with proven ability to work with First Nation communities and people.
· Knowledge of Health Centre programs and services, First Nation service delivery, customs and traditional values, and external services and service agencies.
· Ability to work collaboratively in a team environment, with excellent interpersonal and communication skills.
· Experience writing proposals, policies, procedures and reports, along with excellent computer skills and database experience.
· Strong organizational and time-management skills, demonstrated financial and resource-management skills, and demonstrated excellence in conflict resolution, mediation and problem solving.
· Solid professional ethics; ability to work with confidential and sensitive information; and ability to be a positive role model and community citizen while practicing healthy lifestyle choices and a holistic world view.
· A clear Police Records Check with Vulnerable Sector Check; Mental Health First Aid; ASIST Certification; and First Aid/CPR.
· A valid Class G Ontario Driver’s Licence, unrestricted access to a vehicle, ability to travel, and $1 million automobile insurance. Ability to understand and speak the language is a definite asset.
