- The Nurse Navigator/Case Manager exemplifies the values of Compassion, Accountability, Respect and Simplicity. He/she sets the clinical stage for the patient to have a positive, efficient, effective and secure experience.
- The Nurse ensures smooth health care system navigation and effective case management as required for those patients who require the clinical intervention of a Nurse.
- The Nurse will utilize the nursing process, assess the needs of the patient and the family and facilitate interventions to improve health outcome in collaboration with all team members.
- Regular Clinic hours: Monday, Tuesday, Wednesday and Friday 8:30am-4:30pm, and Thursday 12:00pm-8:00pm
- After-hours Clinic: Two after-hours Clinics 8:30am-12:30pm on Saturday and Sunday once a month
- Vacation and time off coverage for other front office support staff and nurses
- Daily Clinical Duties
- Follow the privacy and confidentiality act with extreme care.
- Triages phone calls and walk-ins and determines urgency and advise appropriately.
- Schedules patient appointments and follow-ups as needed.
- Arranges for prescription renewals and updates as per physician’s direction.
- Assists with education and training of recent staff and residents/medical students as required.
- Pleasantly greets patients and brings them into examination rooms as per physician’s direction.
- Takes patients’ weight and other vital signs (blood pressure, heart rate, height, temp etc), prepare infants (weighing and measuring) and input values into EMR at all appropriate visits.
- Prepares and follows up on the complex tests and requisitions as required for the patients and provides appropriate instructions and follow up (as required).
- Completes chronic disease templates as well as well baby templates for each clinic encounter
- Provides wound assessment, application of dressings and splints, staple and suture removal, preparing for and assisting in procedures (e.g. dermatological biopsies and excisions, ear syringing) as ordered by the physician. Mentors and delegates to other staff as required following College of Nurses requirements.
- Assists in or performs home visits for providing primary health care as per physician’s direction.
- Follows protocol to receive/track and communicate International Normalization Ratios (INR) reports received from labs. Mentors and delegates to front office staff as appropriate.
- Provides interpretation of more complex lab results and diagnostic tests as delegated and instructed by the physician.
- Provides mentorship as required to the front office clinical assist role for antenatal and postnatal follow up.
- Provides mentorship as required to the front office clinical assistant role, to provide, monitor, track and record all preventative care and screening to include Immunizations, TB skin tests, Allergy injections, Regular Injections (e.g. B12, Testosterone), Flu shots, Mammograms, Bone Mineral Density Tests, PSA testing, cervical cancer screening, referrals for colonoscopy, and Fecal Occult blood screening.
- Keeps track of Nurse’s billings.
- Assists in quality improvement projects and research as per physician’s direction.
- Additional Clinic responsibilities, sterilization and cleaning as needed.
- Ensures that sterilizer, procedure rooms and other medical equipment are maintained in keeping with Health & Safety standards
- Maintain inventory count for all aspects of clinical practice and ensures that exam rooms are maintained and stocked regularly
- Case Management
- Maintains and updated Recall and Reminder lists on a monthly basis
- Referrals are tracked regularly and outstanding reports are collected in a prompt manner
- Reviews and updates all patient forms (OCEAN, Health Myself, Laboratory requisitions) prior to to patient visits, and updated weekly for the following week
- Follows up on chronic disease patients completing templates as required to assist the physician in the ongoing management of all chronic disease patients (e.g. Diabetes, Asthma, COPD, Cholesterol management, CKD). Identifies and completes the follow-up required to meet identified targets for effective care delivery.
- Performs and manages blood pressure follow-ups as delegated by MD following the Canadian Hypertension guidelines for target BP.
- System Navigation
- Reviews the admission and discharge reports of patients to ensure an effective follow up plan of care is implemented to include liaison with community supports are implemented.
- Follows the progress of all palliative patients (with the physician, care coordinators, hospital) to ensure an effective plan of care is in place.
- Follows up on and books the complex referrals for special testing/physician consults/referrals/Allied Health Referrals/diagnostic investigations as requested by the physician. Provides instruction to patients for complex investigations.
- Assists patients and families to arrange community support services and completes the appropriate referrals, as needed.
- Follows up with the London Middlesex Public Health as required and collaborates with the front office staff to ensure that all updates are posted (e.g. SARS/Ebola/Flu/COVID etc).
- Clinical Support
- Works with front office staff as required to facilitate effective flow of patients.
- Assists the front office staff to stock exam rooms and nursing station as required with medical supplies to optimize efficiency and flow for all examinations/procedures.

- RN or RPN current license and member in good standing the with College of Nurses of Ontario and BCLS/CPR certification required.
- Member of RPNAO or RNAO with evidence of liability insurance, required.
- 3 to 5 years’ experience in a Clinic and or family practice office or equivalent, required.
- Participates in the College of Nurses Quality Assurance program, required.
- Demonstrates excellent interpersonal skills to include problem solving, flexibility, conflict resolution, organizational and communication skills required.
- Proficiency in use computers and medical practice equipment required.
- Professional attire required (scrubs and closed toe shoes are required).
- Demonstrates ability to maintain confidentially for all information pertaining to the work environment required.
- Current police check (within last 6 Months) required.
- Must be adept in the use of Microsoft Office 2010 and PS Suites EMR software
- Required to work in a very busy fast paced environment
- Required to work with patients who may have significant emotional and physical challenges.
- Normal working hours as per physician’s schedule (see above).
- Able to work in a fast-paced environment required to be on your feet for 80 percent of the day.
- Able to work on a computer or phone for extended periods of time.
- Able to use effective body mechanics to assist patients onto the examining table or to the bathroom daily.
- May require heavy lifting on a daily basis.
