Your Opportunity

Social Work at the NECHC provides a unique opportunity to work to your full scope of practice. This position requires both a high degree of independent critical thinking as well as the ability to work collaboratively within a multidisciplinary team. You will have the opportunity to provide advanced level clinical Social Work services including crisis intervention and counselling with patients and families in the ambulatory care setting.

You will respond to the instrumental, emotional, and social needs of clients and families using a variety of psychosocial interventions including counselling and psychoeducation. An essential part of the role is to identify and address/respond to structural barriers, social justice and health equity issues that impact the social determinants of health of individual clients and families.

The Social

Worker works with clients to achieve recovery-oriented goals and social wellbeing. Working from a concurrent capable perspective, you will be required to prioritize workload, formulate assessments and create intervention plans. These plans include psychosocial interventions, comprehensive risk assessment, and recommendations and provision of referrals for ongoing treatment and/or discharge planning.

The Social worker will demonstrate exceptional communication skills with patients, family members and physicians.

Description

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families.

Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Required Qualifications

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Additional Required Qualifications

Minimum 5 years experience working in Acute Care as a Social Worker. Previous experience with Pediatric Population required. Effective time management and prioritization skills are required. Conflict resolution and strong communication skills are required. Must be able to work independently and within a team workplace. Knowledge of relevant legislation to direct practice and assists members of the health care team in delivering services within legislative requirements: e.g.

Mental Health

Act (MHA), Adult Guardianship & Trustee Act (AGTA), Personal Directives Act, Child, Youth and Family Enhancement Act (CYFEA). Ability to problem solve and demonstrate critical thinking skills is required. Able to mediate and support resolution of conflict situations within the family system and between client/family and health care team.

Knowledge and ability to use computer applications such as Microsoft Office, Excel, and Outlook required. Must be able to tolerate long periods of sitting or standing.

Preferred Qualifications

Completion of a Master’s Degree in Social Work preferred.

Experience working in an Ambulatory setting as a Social Worker, 5 years’

experience working in an acute care setting, authorization to perform psychosocial interventions, Non-violent Crisis Intervention training, Applied Suicide Intervention skills training or equivalent, Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) and previous experience with Connect Care preferred.
